One of my young adult Angel Fish quit eating and showed clear signs of an internal parasite.
I have had good luck using Metronidazole for this kind of problem in the past, but that medication has become rather hard to find.
In the store, something called "General Cure" didn't sound promising to me, so I ignored it at first. After failing to find anything else on the shelves that seemed suitable, I finally looked at the back of this box and, voila, one of the listed ingredients was Metronidazole!
It took a few days after completing the two dose treatment for the Angel to resume eating, but he did and the rest of his symptoms quickly disappeared as well. A year later, he is full grown, remains as healthy, active, and large as his "brother" in the same tank. (There was a day when an Angel Fish "hunger strike" was a death knell.)
This is much more expensive than plain Metronidazole, but it is available at my local PetSmart, and it may even be more effective. Well worth it's high price for valuable fish.

General Cure is one medication that I always have on hand. I have natural treatments for parasites that I try first. If I don't see improvements in a few days, then I use General Cure. As another reviewer wrote, I saw improvement after just one treatment when my Leporinus had hole-in-the-head (at least, what looked like hole-in-the-head). By the time treatments was over, I could barely see the wound. My son recommended it to me (worked for his Peacock cichild, too). Good thing I had some on hand (I had forgotten that it treats hole-in-the-head).
Price can be a pro - or con - depending on how much you value your fish. I can't even find a replacement for my leporinus - so treatment was worth the expense. I had to treat 75 gallons. He is too big to hospitalize (about 11" long). One of my little cory cats may have hole in the head now (or she may have scarped her head on rocks), but I am only using natural treatments for her. She seems to be doing OK.
Pros: works great, price, easy to administer

I bought this product to cure hole in the head disease that my Tiger Oscar had. I used both doses in my 55 Gallon but the Oscar looked 100% better after just the first dose. This stuff works incredibly well and saved my fish!
